    On the nature of X-Ray Flashes in the SWIFT era

    X-Ray Flashes (XRFs) are soft gamma-ray bursts whose nature is not clear. Their soft spectrum can be due to cosmological effects (high redshift), an off-axis view of the jet or can be intrinsic to the source. We use SWIFT observations to investigate different scenarios proposed to explain their origin. We have made a systematic analysis of the afterglows of XRFs with known redshift observed by SWIFT. We derive their redshift and luminosity distributions, and compare their properties with a sample of normal GRBs observed by the same instrument. The high distance hypothesis is ruled out by the redshift distribution of our sample of XRFs, indicating that, at least for our sample, the off-axis and sub-energetic hypotheses are preferred. Of course, this does not exclude that some XRFs without known redshift could be at high distance. However we find that taking into account the sensitivity of the BAT instrument, XRFs cannot be detected by SWIFT beyond ~ 3. The luminosity distribution of XRF afterglows is similar to the GRB one. This would rule out most off-axis models, but for the homogeneous jet model. However this model predicts a GRB rate uncomfortably near the observed rate of supernovae.     We present the complete on-line catalogue of gamma-ray bursts observed by the two Wide Field Cameras on board \sax in the period 1996-2002. Our aim is to provide the community with the largest published data set of GRB's prompt emission X-ray light curves and other useful data. This catalogue (BS-GRBWFCcat) contains data on 77 bursts and a collection of the X-ray light curves of 56 GRB discovered or noticed shortly after the event and of other additional bursts detected in subsequent searches. Light curves are given in the three X-ray energy bands (2-5, 5-10, 10-26 keV).     Properties of X-Ray Rich Gamma Ray Bursts and X-Ray Flashes detected with BeppoSAX and Hete-2

    We study the spectrum of the prompt emission and the X-ray and optical afterglow fluxes of 54 X-Ray Rich Gamma Ray Burst (XRRs) and X-Ray Flashes (XRFs), observed by BeppoSax and HETE-2. A comparison is then performed with classical Gamma Ray Bursts (GRBs). The goal of this paper is to investigate the nature of XRRs/XRFs, as high redshift GRBs or off-axis GRBs, analyzing both their prompt and afterglow properties. We find that the XRR/XRF spectral indexes of the Band function are similar to those of classical GRBs, whereas the peak energy is lower by a factor of 4. We study the optical and X-Ray afterglow properties of the XRRs/XRFs; in particular we analyze the XRR 011030 afterglow. We find that the X-ray and optical flux distributions and the lightcurves of the XRRs/XRFs sample are consistent with those of classical GRBs; in particular, they show evidence of a break and no rising temporal slope. We compare these results with the afterglow predictions of the high redshift scenario, where XRFs are GRBs at higher redshift and of the off-axis scenario, where the observed differences are due to viewing angle effects. In this last framework, we consider jets with a homogeneous, a -2 power-law shaped and a Gaussian luminosity angular distribution. We find that the high redshift scenario can explain some events but not the total sample of XRRs/XRFs. The off-axis model may be consistent with our findings when a homogeneous jet is considered. However, given the uncertainties on the selection effects in our sample, a Gaussian jet viewed at small angles from the Gaussian core and a power-law shaped cannot be ruled out.Comment: 18 pages, 12 color figures, 1 b/w figures, accepted to Astronomy and Astrophysic

    A statistical study of gamma-ray burst afterglows measured by the Swift Ultra-violet Optical Telescope

    We present the first statistical analysis of 27 UVOT optical/ultra-violet lightcurves of GRB afterglows. We have found, through analysis of the lightcurves in the observer's frame, that a significant fraction rise in the first 500s after the GRB trigger, that all lightcurves decay after 500s, typically as a power-law with a relatively narrow distribution of decay indices, and that the brightest optical afterglows tend to decay the quickest. We find that the rise could either be produced physically by the start of the forward shock, when the jet begins to plough into the external medium, or geometrically where an off-axis observer sees a rising lightcurve as an increasing amount of emission enters the observers line of sight, which occurs as the jet slows. We find that at 99.8% confidence, there is a correlation, in the observed frame, between the apparent magnitude of the lightcurves at 400s and the rate of decay after 500s. However, in the rest frame a Spearman Rank test shows only a weak correlation of low statistical significance between luminosity and decay rate. A correlation should be expected if the afterglows were produced by off-axis jets, suggesting that the jet is viewed from within the half-opening angle theta or within a core of uniform energy density theta_c. We also produced logarithmic luminosity distributions for three rest frame epochs. We find no evidence for bimodality in any of the distributions. Finally, we compare our sample of UVOT lightcurves with the XRT lightcurve canonical model. The range in decay indices seen in UVOT lightcurves at any epoch is most similar to the range in decay of the shallow decay segment of the XRT canonical model. However, in the XRT canonical model there is no indication of the rising behaviour observed in the UVOT lightcurves.Comment: 16 pages, 9 figures, accepted MNRA

    Italian recommendations on enzymatic debridement in burn surgery.

    Abstract Introduction Nexobrid®, a bromelain-based type of enzymatic debridement, has become more prevalent in recent years. We present the recommendations on enzymatic debridement (Nexobrid®)'s role based on the practice knowledge of expert Italian users. Methods The Italian recommendations, endorsed by SIUST (Italian Society of Burn Surgery), on using enzymatic debridement to remove eschars for burn treatment were defined. The definition followed a process to evaluate the level of agreement (a measure of consensus) among selected experts, representing Italian burn centers, concerning defined clinical aspects of enzymatic debridement. The consensus involved a multi-phase process based on the Delphi method. Results The consensus panel included experts from Italy with a combined experience of 1068 burn patients treated with enzymatic debridement. At the end of round 3 of the Delphi method, the panel reached 100% consensus on 26 out of 27 statements. The panel achieved full, strong consensus (all respondents strongly agreed on the statement) on 24 out of 27 statements. Discussion The statements provided by the Italian consensus panel represent a "ready to use" set of recommendations for enzymatic debridement in burn surgery that both draw from and complete the existing scientific literature on the topic. These recommendations are specific to the Italian experience and are neither static nor definitive. As such, they will be updated periodically as further quality evidence becomes available

    Rise and fall of the X-ray flash 080330: an off-axis jet?

X-ray flashes (XRFs) are a class of gamma-ray bursts (GRBs) with a peak energy of the time-integrated spectrum, , typically below 30 keV, whereas classical GRBs have of a few hundreds of keV. Apart from and the systematically lower luminosity, the properties of XRFs, such as their duration or spectral indices, are typical of the classical GRBs. Yet, the nature of XRFs and their differences from GRBs are not understood. In addition, there is no consensus on the interpretation of the shallow decay phase observed in most X-ray afterglows of both XRFs and GRBs. Aims. We examine in detail the case of XRF 080330 discovered by Swift at redshift 1.51. This burst is representative of the XRF class and exhibits an X-ray shallow decay. The rich broadband (from NIR to UV) photometric data set we collected during this phase makes it an ideal candidate for testing the off-axis jet interpretation proposed to explain both the softness of XRFs and the shallow decay phase. Methods. We present prompt -ray, early and late NIR/visible/UV and X-ray observations of the XRF 080330. We derive a spectral energy distribution from NIR to X-ray bands across the shallow/plateau phase and describe the temporal evolution of the multi-wavelength afterglow within the context of the standard afterglow model. Results. The multiwavelength evolution of the afterglow is achromatic from ~102 s to ~8104 s. The energy spectrum from NIR to X-ray is reproduced well by a simple power-law, , with = 0.790.01 and negligible rest-frame dust extinction. The light curve can be modelled by either a piecewise power-law or the combination of a smoothly broken power law with an initial rise up to ~600 s, a plateau lasting up to ~2 ks, followed by a gradual steepening to a power-law decay index of ~2 until 82 ks. At this point, a bump appears to be modelled well with a second component, while the corresponding optical energy spectrum, , reddens by = 0.260.06. Conclusions. A single-component jet viewed off-axis can explain the light curve of XRF 080330, the late-time reddening being due to the reverse shock of an energy injection episode and its being an XRF. Other possibilities, such as the optical rise marking the pre-deceleration of the fireball within a wind environment, cannot be excluded definitely, but appear to be contrived. We exclude the possibility of a dust decreasing column density being swept up by the fireball as explaining the rise of the afterglow.Peer reviewe

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy

    IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis. OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer. DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery. EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections. MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable. RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P < .001), and stenotic lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03). CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients
